The Position:

Join a values-driven team providing high-quality, individualised Support Coordination services while leading and mentoring a dedicated team of Support Coordinators and Recovery Coaches.

Duties of the role will include (not limited to):

  • Provide coaching, mentoring, and professional guidance to team members to support quality service delivery and adherence to organisational policies, procedures and documentation requirements;
  • Develop, implement, monitor and review individual client plans, including behaviour support and complex health plans in consultation with key stake holders;
  • Identify and implement strategies to ensure continuous improvement in team practice and service delivery;
About You:

Minimum Qualifications:

  • Proof of Australian Working Rights
  • Current Drivers License & Car Insurance
  • A minimum Tertiary qualification in Mental Health or Disability Services, or a Diploma and/or Cert IV in Mental Health / or relevant area of study and minimum of 3 plus years’ experience working in a similar position.

Candidate will also either have or be willing to obtain the following:

  • NDIS Worker Screen Clearance
  • NDIS Orientation Module Certificate
  • Working with Children Card
  • First Aid Certificate
